1. Click the Mastercard Link tab
   • Accept Terms & Conditions. Check the box next to “I have reviewed and accept the Terms and Conditions” and click Next.
2. Card Information and Verification
   • Input Credit Card Number, if not already populated. 
   • Enter the last 4-digits of the Primary Cardholder’s Social Security Number.
   • Select the Primary Cardholder's date of birth.
   • Enter the first 5-digits of the Zip code associated with the Primary Cardholder’s address.
   • Click Next.
3. Profile Information – information must match what we have on file.
   • Input the following information:
     • First and Last Name of Cardholder
     • Card nickname (Optional)
     • Phone Number and Email Address and Verify Email Address
     • Click Next
4. Enrollment Success
   • Click OK 
   • Account Summary page should load.
